Please
familiarize yourself with the policies below and then proceed to print
our application. By clicking the 'Agree' button at the bottom of this
page, you are legally accepting and agreeing to abide by all stated policies

PAYMENTS
- Total
program cost (listed under each program) is due 45 days prior to the
start date of the program.
- PayPal,
Visa,
MasterCard, American Express, Bank Transfer are all accepted.
- 7%
DISCOUNT for cash payment (in advance)
- Clinics
and/or One on One training sessions will not be booked without pre
payment of the full amount due.
- As
a Canadian company, we can only bill in Canadian funds. We will estimate
as closely as possible using daily bank rates, when entering amounts
on credit cards.
- For
the most accurate U.S. payment method, please pay by cheque.
- As
a Canadian company, we must legally charge HST on all school
enrolments for schools located in Canada. Non Canadian residents are
able to contact Canada Customs and receive a reimbursement for a
portion of taxes paid on purchases while in Canada.
- All
applications MUST be accompanied by a deposit equal to 30% of the total
program cost.
- The
remaining 70% of the program cost may be paid by any number of post
dated cheques. These cheques MUST be dated NO LESS than 45 days prior
to the program start date.
- Applications
arriving less than 45 days prior to the start of a program MUST be accompanied
by full payment.
- Please
do not fax in applications that are going to be mailed with payments.
Any application that arrives without an accompanying cheque or credit
card will be discarded. We cannot hold or reserve spaces without payments.
- Cheques
or money orders are to be made payable to Level Playing Field Sports
Concepts Inc.
- N.S.F
cheques are charged a $50.00 administration fee.
CANCELLATIONS
& REFUNDS
- All
school cancellations up to 60 days prior to the program start date, will be
subject to a $75 administration fee.
- Any
school cancellation less than 60 days prior to the program start date, will
result in the loss of all monies paid.
- Credit
towards a future program will be issued, for any school cancellation with 60
days or less notice, only if an ORIGINAL doctors certificate of health,
showing cause for the students non attendance, is provided within 30
days of the actual date of notification to our office. No cash will
be refunded.
- Absolutely
no refunds or credits will be issued for any cancellation, no show or
non confirmed booking time of a Clinic
or One on One training session
- Absolutely
no refunds or credits will be issued once a program has started, for
any reason.
- All
refunds or credit notes will be issued 15 days after the completion
date of the program
MISCELLANEOUS
- Enrolment
confirmation and itineraries will be available through our web site.
A password will be issued (providing payment and PDC's have been received)
60 days prior to the programs start date. If the student does not have
web access, this information will be mailed.
- All
billeting arrangements will be confirmed 2 weeks prior to the start
date of the school, PLEASE DO NOT CALL FOR THIS INFORMATION BEFORE THIS
